VIM Berkshires provides free health care to adults who are ineligible for insurance and income-qualified. To determine eligibility for VIM’s services and set up an appointment, please fill out the form below or call 413.528.4014.
Eligibility Requirements
In order to be eligible for VIM’s services you must meet the following four requirements:
You are ineligible for Health Insurance.
You must be ineligible for health insurance to become a patient at VIM. All patients will also be directed to Advocacy for Access for a second insurance eligibility screening. Please also note: Dental services are only available to our medical patients.
Your income is below 300% of the Federal Poverty Rate.
A family’s gross income (before taxes) must be at or below the income guidelines listed below. “Family” means nuclear families and their dependents who live together in a household, or family status as defined on the federal tax return.
You are 18 years of age or older.
We only see adult patients. All children are eligible for MassHealth.
You live in the Berkshire region including nearby towns in New York State and Connecticut.
Note: New York State and Connecticut patients must be made aware that if they require treatment outside VIM’s health care center, they may be liable for any costs incurred.

B-A-S-I-C stands for Berkshire Alliance to Support the Immigrant Community. BASIC was formed to support the Berkshire immigrant community. The website lists resources for health and safety, housing, clothing & household goods, food assistance, immigration & legal aid, employment, children & youth, financial assistance, English classes, transportation, banking, higher education, translation, and community.
